The Manatee County Health Department Vital Statistics office issues certified copies of birth certificates for persons who were born in the state of Florida from 1930 through the present. If you were born in Florida, you may be able to get a certified copy of your birth certificate from the health department.

The applicant must be able to submit valid identification. The identification must be in the form of valid/unexpired photo ID* in order to determine the applicant is the person they claim to be. The ID must be intact, in one piece, not torn or in more than one piece.
If you were not born in the state of Florida, you may be able to order a certified copy of your birth certificate through your birth state.
