Under the Federal Driver Privacy Protection Act of 1994, driver license or identification card numbers, names, addresses, telephone numbers, social security numbers, emergency contact information, email addresses and medical or disability information are not public information. Therefore, none of this information will be given out by any of our offices to anyone other than the person on record.
How Can I Obtain Information from My Driver License or Motor Vehicle Record?
- For Driver Record History (both certified and uncertified transcripts can be purchased), create an account online – or scan the QR code to access the site.
- For other Driver Record requests visit flhsmv.gov, complete HSMV Form 90511, sign and mail to:

How Can I Obtain Personal Information from Someone Else's Record?
- Using the forms listed above (HSMV 90510 OR HSMV90511), you may submit your request to the DHSMV; however, the information will only be released if you meet an exemption covered by law. Applicable exemptions are listed on each of the forms. For more information, call (850) 617-2000.
- If you know the other individual’s personal information, you can request a copy of their Driver Record History (transcripts) by creating an account online – or scan the QR code to access the site.
- Parents and guardians have free access to their children’s (age 18 or under) driving records by creating an account online – or scan the QR code to access the site. You will need your child’s driver license number, social security number and date of birth to access their record.
- If you are trying to obtain lienholder information, for businesses only, you may visit the DHSMV’s Online Motor Vehicle Check at services.flhsmv.gov/MVCheckWeb/. Here you can research lienholder information by title number or vehicle identification number (VIN). No owner information is provided.
- If you were involved in a crash and need to request the other party’s insurance information, you must mail a copy of the complete crash report or the Driver Exchange Form to:
