During your application for a Florida driver license, you indicated that you have been convicted of Driving Under the Influence (DUI) multiple times in another state as indicated below.
- Two (2) DUI’s in the last five (5) years
- Three (3) DUI’s in the last ten (10) years
A Florida record has been created for you and it has been updated to reflect this information, which may be viewed by any driver license or tax collector’s office in the State of Florida.
In order to obtain a Florida driver license, you will be required to provide the following to the Driver License Branch. The items below are in addition to any documents required as part of the Federal Real ID Act. Failure to submit the required documents will result in a return visit to our office.
- Proof of completion of out-of-state or Florida DUI School
- Florida insurance card, policy or SR-22 form that specifies Bodily Injury Liability (BIL), Property Damage Liability (PDL) and Personal Injury Protection (PIP) coverage

An Online Application is REQUIRED for all transactions! To complete your driver license or ID card transaction at one of our branches, you must complete the Online Driver License & ID Card Application (https://taxcollector.jotform.com/210103795736152) – or scan the QR code to access the application – prior to your appointment. Do not print the application; an associate will access your electronic submission during your appointment. Applications are valid for 90 days.
