When reinstating a DUI-related suspension or revocation, you must show proof of enrollment or completion in DUI School, which will be scanned and subject to verification. (Note: When applying for a hardship license for DUI revocation at a BAR Office, you must show completion [not enrollment].)
There are two levels of DUI School based on prior DUI convictions. All costs involved in these programs will be incurred by you.
- LEVEL 1: Required for first-time DUI convictions
- LEVEL 2: Required for multiple DUI convictions
If “additional treatment” is stipulated on your completion certificate, you will be required to attend substance abuse treatment. (Proof of enrollment or completion of substance abuse treatment is not necessary when reinstating your license. However, should you not comply, your license will be cancelled. See below for more details.)
Special Supervision Service: If you have had multiple DUI convictions and have been granted a hardship license, you must take part in special supervision in addition to Level 2 DUI School. (This comes after the hardship license has been issued, so it’s not necessary to verify enrollment when reinstating your license. However, should you not comply, your license will be cancelled. See below for more details.)
1. DUI SCHOOLS
Our local DUI School is hosted by the State College of Florida Traffic Safety Institute (941) 752-5286. However, you may choose to complete your DUI School with another acceptable provider. Providers can be found at: flhsmv.gov/ddl/dui_prog.html or in the phonebook.
Please note the following:
- Online DUI school is not acceptable
- Out-of-country DUI School is not acceptable
- Military DUI School must be from Prevent or Impact
- If you are a Florida resident, you cannot go out-of-state to complete DUI School
- Out-of-state DUI School is only acceptable if you have moved to another state (the DUI School must be licensed by the licensing authority in their new state)
If you live or work part-time in another state, you may request an exception. Call (850) 617-2000 for assistance.
2. FAILURE TO COMPLY
If you fail to comply with DUI School (including treatment and/or supervision), your driving privilege will be cancelled.
DUI SCHOOL - You have 90 days to complete DUI School after reinstating your license. If you fail to complete it within that time period, your license will be cancelled until the course is successfully completed. To reinstate the cancellation, you must provide proof of completion of DUI School, which will be scanned and subject to verification.
TREATMENT - If the DUI Program notifies the Division of Driver Licenses in Tallahassee that you failed to complete the Substance Abuse Treatment Program, your license will be cancelled. And, depending on prior cancellations, you would be required to submit:
- First Cancellation – You must provide form HSMV 77096 dated not more than 30 days ago*. The form must indicate: (1) you have re-entered and are currently participating in treatment, or (2) you have completed the Substance Abuse Treatment Program. Form HSMV 77096 will be scanned and subject to verification.
- Second Cancellation - You must provide form HSMV 77096 dated not more than 30 days ago*. The form must indicate that you have completed the Substance Abuse Treatment Program. (You will not have the option of showing proof of current participation if it is your second or subsequent cancellation for this reason.) Form HSMV 77096 will be scanned and subject to verification.
*If the form HSMV 77096 is dated more than 30 days ago, it will not be accepted.
SUPERVISION - If you volunteered to participate in the DUI Supervision Program as a condition of early reinstatement (hardship license) but you fail to attend and participate, your license will be cancelled. You will be required to wait out the cancellation period.
3. ACCEPTABLE METHODS OF PAYMENT AND ESTIMATED FEES
Our office accepts cash, check, debit cards, and credit cards. A $2.50 processing fee for debit cards and a 2.5% processing fee (minimum of $2.50) for credit cards will apply. Our office does not retain any portion of this fee.

4. INVALID LICENSE INFORMATION
Is your license currently suspended, revoked and/or cancelled? If your driver license is suspended, revoked and/or cancelled, driving any type of vehicle may result in:
- ARREST by law enforcement
- TOWING of your vehicle
We urge you to respect the law and make arrangements other than driving a vehicle to avoid the unfortunate events above. A free public phone and computer are provided in the lobby to call for a ride from our office or to access the Manatee County Area Transit (MCAT) website.
Note: A minor child operating a motor vehicle with a learner license must be accompanied by another adult with a valid license (non-suspended).
How can you get your license back once it's reinstated? If your license was retained by one of our branches due to a suspension, revocation and/or cancellation, you may be able to have it returned for no additional fee once it has been reinstated. Licenses retained by our office are only kept for a short period of time before they are destroyed.
If you are interested in getting your license back, you must do the following within 1-2 days after your license was retained.
- Return to the branch that retained your license.
- Print the screen from https://mydmvportal.flhsmv.gov/home/en/publicweb/dlcheck that shows your license is “VALID” that day.
- Provide the screen print to the receptionist.
If your license has been destroyed or you opt to purchase a replacement license at the time you reinstate your driving privilege, you would be subject to the $31.25 replacement fee.
5. ADDITIONAL INFORMATION
You can sign up for electronic notifications when citations and/or sanctions are added to your record online - or scan the QR code to access the site.
When reinstating, if you need a replacement license or ID card, you will be required to submit documents that establish your identity pursuant to the Real ID Act (e.g. US birth certificate, US passport, name change(s) documents, USCIS (INS) documents, Social Security Card and 2 proofs of residential address). You may be exempt from providing these documents if you have already been issued a Real ID compliant Florida license or ID card and no changes are needed. An Online Application is REQUIRED for all transactions! To complete your driver license or ID card transaction at one of our branches, you must complete the Online Driver License & ID Card Application (https://taxcollector.jotform.com/210103795736152) – or scan the QR code to access the application – prior to your appointment. Do not print the application; an associate will access your electronic submission during your appointment. Applications are valid for 90 days.
