1. WHAT IS A LAW ENFORCEMENT/SERVICEMEMBER BLOCK?
Your personal information contained in your motor vehicle or driver license/identification card records are blocked (withheld) from the public. As defined in the Florida Public Records Law, Section 119.0712(2), Florida Statutes, personal information in a motor vehicle record includes, but is not limited to driver identification (license) number, name, address, telephone number, and medical or disability information.
2. HOW CAN YOU GET A LAW ENFORCEMENT BLOCK PLACED ON YOUR RECORD?
To request a law enforcement block on your motor vehicle and driver license/identification card records, the following must be completed (if you wish to block your personal information with any other agency, you must contact that agency to do so):
- Properly complete HSMV 96020 (https://www.flhsmv.gov/pdf/forms/96020.pdf) Public Official’s Request to Suppress Records Information form.
- Fax form HSMV 96020 along with a signed** notarized letter on law enforcement letterhead, which states you are a current/former law enforcement officer, your badge # and the date(s) that you were employed to 850.617.3916. (**This letter must be signed by your commanding officer or another individual qualified to sign.)
3. HOW CAN YOU GET A SERVICEMEMBER BLOCK PLACED ON YOUR RECORD?
To request a servicemember block on your motor vehicle and driver license/identification card records, the following must be completed and submitted to the Department of Highway Safety and Motor Vehicles at the address listed on the HSMV 96020 form (if you wish to block your personal information with any other agency, you must contact that agency to do so):
- Properly complete HSMV 96020 (https://www.flhsmv.gov/pdf/forms/96020.pdf) Public Official’s Request to Suppress Records Information form.
- Provide a letter on letterhead from your employer or other documentation (e.g. copy of military ID, DD214 etc.), that confirms your entitlement to the block.
- Submit a written statement attesting to the fact that reasonable efforts have been made to protect your identification and location information from being accessible through other means available to the public.
