1. To Apply For or Maintain a Hazmat Endorsement
Applicants must meet all of the following requirements for an Original CDL with Hazmat, Out of State CDL with Hazmat Transfer, or Florida CDL with Hazmat Renewal:
- Must be a US Citizen or Permanent Resident (Immigrant) at least 21 years of age.
- Must be a Florida resident with proof of Social Security Number that verifies with the Social Security Administration.
- Present documents that establish your identity pursuant to the Real ID Act (e.g. US birth certificate, US Passport, Permanent Resident Card and 2 proofs of residential address in your name). For a complete list of acceptable identity documents, visit flhsmv.gov/whattobring.
- Hold a current Florida Commercial Driver License or complete all requirements for your desired CDL class. Note: A Hazmat endorsement cannot be added to a Commercial Learner Permit (CLP).
- Provide a valid medical certification card (if applicable). For complete medical certification category details, visit fmcsa.dot.gov/faq.
- Complete Entry-Level Driver Training (ELDT) (required for first-time Florida Hazmats only). This training is not offered by our office. For more information, please visit https://tpr.fmcsa.dot.gov/.
- Pass the Hazmat written exam.
- An Online Application is REQUIRED for all transactions! Complete the Online Driver License & ID Card Application (https://taxcollector.jotform.com/210103795736152) – or scan the QR code to access the application – prior to your appointment. Do not print the application; an associate will access your electronic submission during your appointment. Applications are valid for 90 days.
- A Hazmat Application is REQUIRED! Complete the Hazardous Materials Endorsement Application online (flhsmv.gov/hazmat).
- Submit to Fingerprinting and pass a Transportation Security Administration (TSA) background check within 60 calendar days of completing the Hazmat written exam. Note: If this process is not completed within 60 days, you will be required to retake the Hazmat written exam.
2. Length of Hazmat Endorsed CDL & Renewal Information
A Hazmat endorsed Commercial Driver License (CDL) is valid for a period of 4 years, not to exceed 5 years. Hazmat CDL holders can renew up to 12 months in advance.
It is important to begin the renewal process at least 90 days in advance of the expiration date of your license, to avoid possible ineligibility to transport hazardous materials while awaiting the results of the background check.
3. Hazmat Commercial License (CDL) Fees
We will accept cash, check, debit cards, and credit cards. A $2.50 processing fee for debit cards and a 2.5% processing fee (minimum of $2.50) for credit cards will apply. Our office does not retain any portion of this fee.

Note: Fees are non-refundable if you do not pass the background check.
4. Transportation Security Administration (TSA) Background Check Application
Application Approved: The Commercial Driver License (CDL) with Hazmat endorsement will be mailed to you from Tallahassee. This process can take up to 90 days. You can check the status of your application by visiting flhsmv.gov. If your Hazmat endorsement is about to expire, you can contact the CDL Department in Tallahassee at (850) 617-2606 to request an extension letter.
Application Denied: You will be notified by mail and your temporary Hazmat endorsement will be withdrawn immediately. If you are holding a Class A or B license, you will be mailed a new CDL without the Hazmat endorsement. If you are holding a Class C license that does not have a Passenger endorsement, you will be downgraded to a regular Class E license. If you wish to appeal a denial, you must do so by contacting TSA at (833) 848-4759.
